Rural Health Value Catalogue of Value-Based Initiatives for Rural Providers -  Summarizes rural-relevant, value-based programs to help rural leaders and communities identify Department of Health and Human Services (HHS) value-based programs appropriate for rural participation. Covers programs implemented by HHS, primarily by the Centers for Medicare & Medicaid Services and its Center for Medicare & Medicaid Innovation. Recent catalog additions include 1) Community Health Access and Rural Transformation (CHART) Community Transformation and Accountable Care Organization Tracks 2) the Radiation Oncology Model, and 3) the Value in Opioid Disorder Treatment Model. National Rural Health Resource Center Population Health Toolkit - provides visualizations of data from multiple sources that answer questions that rural hospitals and communities have about the health of their communities.

Michigan Primary Care Office 2020 Michigan Primary Care Needs Assessment -  comprehensive and informative view of Michigan’s vulnerable populations, unmet health care needs, health disparities, and health workforce issues in the state. This report provides a statewide overview of Michigan as well as highlighting certain areas, counties, and cities with areas of concern, workforce shortages, and barriers to access health care.
