This comprehensive resource is designed to equip healthcare providers with the tools, guidelines, and educational materials needed to effectively screen for and manage CKD in rural populations. By supporting early detection and improving patient care, the toolkit aims to reduce the impact of chronic kidney disease in underserved communities.
The Rural Chronic Disease Management Toolkit outlines evidence-based and promising strategies for managing a variety of chronic diseases. These approaches can benefit many prevalent chronic diseases affecting rural populations, such as diabetes, chronic obstructive pulmonary disease (COPD), heart disease, arthritis, chronic kidney disease, cancer, obesity, and chronic pain. However, it is important to note that the strategies included in this toolkit do not specifically cover all chronic diseases.
